Premium Health At Home in Laramie and Rawlins, WY is locally owned. We are dedicated to providing premium healthcare services to our community.

Nursing/Home Health Nursing

  • The home health nurse plans and provides nursing care in the home, such as IV therapy, wound care, injections, nutritional teaching, monitoring, and patient/family instructions.
  • An Administrative staff member is on call 24 hours a day.

Home Health Aides

(Services provided under the supervision of an R.N.)
  • Personal care for the patient.
  • Therapeutic exercises as prescribed by a licensed therapist.
  • Assistance in maintaining a clean, healthful home environment.

Medical Social Worker

  • Assistance with long-term plans, obtaining financial aid and other resources
  • Assisted coordinator of community resources.
  • Assessment of social and emotional factors impacting the patient's care.
  • Counseling for patient.

Physical Therapy

  • Help to develop and restore functions of limbs and muscles. A hands-on approach to enhance
    healing and decrease pain.
  • Instruction for patients and family in rehabilitative activities to achieve and maintain maximum

Occupational Therapy

Training and adaptive equipment to help increase patient's independence at home with:
  • Dressing
  • Personal hygiene
  • Meal preparation
  • Activities of daily living

Speech Therapy

  • Speech pathologist for oral motor exercises, swallowing training and speech therapy as well as development of communication, cognitive function, and memory.

Long-Term Care/Home Community Based Waiver Services

The LTC/HCBS Waiver is a medicaid Nursing Home level of care waiver for those persons who meet both financial and medical eligibility.
  • Case management: a process by which an individual's needs are identified; and social and medical services to meet these needs are located, coordinated, and monitored.
  • Eligibility is not determined by this office. Please contact your local Department of Family Services (DFS) to find out the eligibility requirements for this program.


Newborn & Pediatric Nursing

The home health nurse can provide the following for newborns & moms:
  • Daily monitoring for babies who need phototherapy (Bili light)
  • Support with lactation for breastfeeding mothers
  • Premature infant care & monitoring
  • Growth and development education
